Development of robust peer review process

In develoment of a robust peer review process, provider engangement is a critical component. Historically, there is certain reluctance on the part of providers to peer review other providers. This reluctance seems to increase in reverse proportion to the size of the organization. Utilizing a strong foundation of performance improvement as the outcome for peer review seems to allieviate this to some extent but what other methods have you used to successfully invigorate a stagnant peer review process?

Clinical & Operational Improvement Resources

Organizations take various approaches when it comes to their structure for quality and performance improvement.  Where organizations have resources, such as management engineers, that traditionally were more focused on operational improvements than clinical improvements, have you kept them organized separately (for example, in different departments) or have you integrated them?  What are your reasons, benefits, etc., for your chosen course?
