It is amazing to me how QI keeps evolving. I was trained in the Deming Model. Six Sigma is now in vogue. A number of experts have voiced opinions in between. On some level it seems to me that the old world "the customer is always right" is still the best maxim. We just have to understand who the customer is in our little segment of the entire picture. Is one system any better than the next?

Use of quality improvement comparative measures, as stated in the power point, are not without their shortcomings due to the fact that data is dependent on documentation, coding, and billing accuracy. Unfortunately we have found this to be true in the HCO that I am employed. The use of the EMR has helped to improve data collection, but is still only as accurate as the user. How and where do you make allowances for these shortcomings?

I am interested to learn how others view the linkage of hospitals to clinics operated by commercial enterprises, like clinics in Walgreens, Albertsons, and Kroger. Which organizations will adopt responsibility to oversee quality for these? How will these entities affect the quality of more traditional health systems, if at all?

About 2 years ago, the IHI put out a short set of metrics known as Whole Systems Measures, which were thought to be a good global measure of how healthcare systems are doing. Is anybody using them, and if so, do they work?
