This is a good question in that I have not ever seen the topic addressed in "reality" but makes so much sense. My position does not have the HR input that it should, but I try. I feel a plan outline is a good start for the HR department as again from what I see, issues are dealt with on a day-by-day basis as opposed to creating and working a long-term strategy. I don't see best practices implemented.
