I work in healthcare and I strongly support upon hire, random and for cause drug screening. I think there are many professions that should and do perform upon hire drug screens. We do not do random only really do any further testing with reasonable cause to request the employee to allow screening. We cannot force them but can request they do so.

My organization requires all employees within seven days of starting to have a drug screen. After that we can then screen for cause. I do wish that some of the areas that have high known abuse potential did random screening. I am a CRNA and have seen some talented individuals waste away to nothing and perhaps if caught sooner could have been saved, or even not been tempted because they knew that a random screen could come their way.

Our HCO also reguires all employees to have a drug screen prior to starting in their position. We also only screen for cause. In my leadership roles I have had too many experiences with employees who have had performances issues tied to substance abuse and often wish we did random testing to help our colleagues sooner if they have a problem.

Our organization require screening prior to first date of employment. We also have a policy for random testing. We want the employees to report when they are taking prescription drugs which would impact their job performance especially if they are required to operate a motor vehicle or dealing with patients.
